$20 – $27 per hour
Office hours 9:00 am – 5:00 pm with occasional referral coverage.

Position summary

The Intake Office Person receives referrals, verifies eligibility and benefits, gathers required documentation and coordinates the admission visit so patients start care without delay.

Essential duties

  • Answer referral calls from hospitals, facilities, physicians and families.
  • Collect demographics, diagnosis, physician information and insurance details.
  • Verify hospice benefit eligibility and obtain authorizations.
  • Schedule admission visits with the RN and communicate timing to all parties.
  • Build the patient chart and ensure consents and face-to-face documentation are complete.
  • Track referral outcomes and report daily census and admission activity.
  • Protect PHI in every call, fax and email.

Requirements

  • High school diploma or equivalent; one year of medical office or admissions experience.
  • Comfort with EMR data entry, scheduling and insurance verification.
  • Clear phone communication and calm judgment under pressure.
  • Background clearance and negative TB screening.

Preferred

  • Hospice or home health intake experience; knowledge of the Medicare hospice benefit.
  • Bilingual English/Spanish.
