Position summary
The Hospice RN completes admissions and comprehensive assessments, develops and updates the plan of care with the interdisciplinary team, and coordinates all care delivered to an assigned caseload.
Essential duties
- Complete admission visits, initial and comprehensive assessments within required timeframes.
- Develop, update and individualize the plan of care with the patient, family and IDG.
- Manage pain and symptoms, obtain physician orders and adjust medications as authorized.
- Supervise LPN and aide visits and complete required supervisory documentation.
- Teach families to give medications, recognize changes and use comfort kits.
- Provide death visits, pronouncement where permitted, and bereavement handoff.
- Keep documentation compliant with Medicare hospice Conditions of Participation.
Requirements
- • Current unencumbered Arizona RN license.
- • One year of nursing experience; hospice, palliative or home health strongly preferred.
- • Current CPR certification and negative TB screening.
- • Valid Arizona driver license, reliable vehicle and current auto insurance.
- • Level One Fingerprint Clearance Card (or eligibility to obtain).
Preferred
- • CHPN certification.
- • Experience with electronic hospice charting and IDG facilitation.
