Licensed Practical Nurse (LPN)

Hands-on comfort care in the patient's home, under RN supervision.

Pay range
$26 – $34 per hour
Plus mileage reimbursement and shift differential for evenings and weekends.

Position summary

The LPN delivers direct hospice nursing care under the supervision of a Registered Nurse, following the plan of care established by the interdisciplinary team and the attending physician.

Essential duties

  • Provide skilled nursing visits in private homes, group homes and assisted living facilities.
  • Administer medications and treatments as ordered and document accurately in the clinical record.
  • Observe and report changes in condition, pain level and symptoms to the case manager RN.
  • Reinforce family and caregiver teaching on comfort measures, medication use and safety.
  • Follow infection control, emergency preparedness and HIPAA requirements at every visit.
  • Participate in interdisciplinary group meetings and on-call rotation as scheduled.

Requirements

  • • Current unencumbered Arizona LPN license.
  • • Current CPR certification and negative TB screening.
  • • Valid Arizona driver license, reliable vehicle and current auto insurance.
  • • Level One Fingerprint Clearance Card (or eligibility to obtain).
  • • Ability to pass background screening and OIG/SAM exclusion checks.

Preferred

  • • One year of hospice, home health or long-term care experience.
  • • Bilingual English/Spanish.
