Position summary
The LPN delivers direct hospice nursing care under the supervision of a Registered Nurse, following the plan of care established by the interdisciplinary team and the attending physician.
Essential duties
- Provide skilled nursing visits in private homes, group homes and assisted living facilities.
- Administer medications and treatments as ordered and document accurately in the clinical record.
- Observe and report changes in condition, pain level and symptoms to the case manager RN.
- Reinforce family and caregiver teaching on comfort measures, medication use and safety.
- Follow infection control, emergency preparedness and HIPAA requirements at every visit.
- Participate in interdisciplinary group meetings and on-call rotation as scheduled.
Requirements
- • Current unencumbered Arizona LPN license.
- • Current CPR certification and negative TB screening.
- • Valid Arizona driver license, reliable vehicle and current auto insurance.
- • Level One Fingerprint Clearance Card (or eligibility to obtain).
- • Ability to pass background screening and OIG/SAM exclusion checks.
Preferred
- • One year of hospice, home health or long-term care experience.
- • Bilingual English/Spanish.
