$32 – $42 per hour ($66,000 – $87,000 annualized)
Mileage reimbursed; licensure supervision hours available.

Position summary

The MSW completes psychosocial assessments, provides counseling to patients and families, connects them with community resources and advocates for safe, dignified care and discharge planning.

Essential duties

  • Complete psychosocial assessments and update the psychosocial plan of care.
  • Provide short-term counseling to patients, caregivers and families, including grief support.
  • Arrange community resources: benefits, transportation, long-term care placement, funeral planning.
  • Assess for abuse, neglect, exploitation and safety risks and complete mandatory reporting.
  • Support advance directive conversations and goals-of-care discussions.
  • Contribute to IDG and coordinate with bereavement services after a death.

Requirements

  • Master's degree in Social Work from an accredited program.
  • Arizona LMSW or LCSW (or eligibility) as required for the assignment.
  • Valid Arizona driver license, reliable vehicle and current auto insurance.
  • Negative TB screening and background clearance.

Preferred

  • Hospice, palliative or medical social work experience.
  • Bilingual English/Spanish.
